Sutter Health Plan Now Offers Access to Sutter Sync

Managing high blood pressure or high cholesterol doesn’t stop when a doctor’s appointment ends. Through Sutter Sync™, eligible Sutter Health Plan members can stay connected to their care team, monitor their health from home and receive personalized support between visits.

Sutter Sync is a no-cost digital care program that combines pharmacist-led care, home monitoring and health education through My Health Online. Designed to make managing chronic conditions easier, the program helps eligible members with high blood pressure, high cholesterol or both — with heart failure support coming soon — stay connected to care from wherever they are.

“Sutter Sync gives members added support between appointments, when questions and challenges can often arise,” said Dr. Robert Bixler, chief medical officer at Sutter Health Plan. “By combining digital tools, connected health data and personalized clinical guidance in My Health Online, we’re making it easier for members to stay engaged in their care and achieve better health outcomes.”

Members begin with a referral from a participating Sutter Health primary care physician and enroll through My Health Online. Once enrolled, they connect with a Sutter Sync Pharmacist who reviews medications, discusses treatment goals and provides individualized guidance based on blood pressure readings or lab results.

For members with high blood pressure, at-home readings are automatically sent to their My Health Online chart via smart monitors. Pharmacists review trends, work with members to optimize medications as needed, and document care activity in Epic, Sutter’s integrated electronic health record, helping keep providers informed between visits.

Since March 2025, more than 11,000 patients have enrolled in Sutter Sync. Among participating hypertension patients, 90% achieved blood pressure control within six months, demonstrating the impact of personalized support between visits.
